10 Questions to Ask Any Home Care Agency (and the Answers That Should Worry You)

Home care in North Carolina is a licensed industry, but quality varies enormously, and the glossiest brochure tells you nothing about who actually shows up on Tuesday morning. These ten questions separate the professionals from the rest.

1. "Are you licensed as a home care agency with NC DHSR, and can I verify it?"

Every legitimate home care agency in North Carolina must hold a state license from the Division of Health Service Regulation, even for non-medical care. Worry if: they dodge, say licensing "doesn't apply" to companion care (it does for agencies), or you cannot find them in the state lookup.

2. "Are your caregivers employees or independent contractors?"

Worry if: contractors. With W-2 employees, the agency carries payroll taxes, workers comp, and liability. With 1099 contractors or "registries," much of that risk can land on your family.

3. "What background checks do you run, and how often?"

The right answer includes criminal background checks (required in NC), plus reference checks, and ideally abuse registry checks. Worry if: the answer is vague, or checks are run once at hiring and never again.

4. "Are you insured and bonded, and can I see the certificate?"

Worry if: any hesitation. Professionals email you the certificate of insurance the same day without being offended.

5. "What is your full pricing, including minimums, and can I have it in writing?"

Most Charlotte-area agencies charge roughly $26 to $36 per hour depending on care level, and almost all have a 3-hour minimum per visit. Worry if: they will not quote a rate until a salesperson visits your home, or the quote excludes weekend, holiday, or "level of care" surcharges you discover later.

6. "What happens when my caregiver calls out sick?"

The real test of an agency is its bench. Worry if: there is no clear answer about backup coverage. A no-show morning with a parent who cannot bathe alone is exactly the crisis you are paying to avoid.

7. "How do you match caregivers, and what if the fit is wrong?"

Good agencies have a defined matching process and replace a bad fit without drama or fees. Worry if: "we send whoever is available."

8. "What training do your caregivers get, especially for dementia?"

If your parent has any memory loss, ask specifically about dementia training. Worry if: the answer is a generic orientation video.

9. "Who supervises the care, and how will you communicate with our family?"

Look for scheduled supervisor check-ins, a care plan you can read, and a way to see visit notes. Worry if: communication depends entirely on the caregiver texting you.

10. "How high is your caregiver turnover?"

The industry's open secret: caregiver turnover nationally is very high, and constant caregiver churn is the top complaint families have. No agency has zero turnover, but honest ones know their number and will discuss how they retain people. Worry if: the question annoys them.
